Uniform Order Form

Please complete the order form below to order uniform items for either new employees or replacement items.

To purchase a variety of additional items, please visit the Employee Uniform Store.

Uniform Tops:
Employees receive 3 uniform tops each year with a choice of any combination of polos and/or t-shirts. Please make sure your total tops ordered does not exceed a quantity of 3.

Outerwear:
A winter coat will be provided to Zoo employees with a choice between Chore Coat and 3-in-1 Coat. Please only order 1 coat. Chore Coats are kept in stock at the Zoo, while the 3-in-1 Coat is a special order. To order the 3-in-1, choose Chore Coat in the form, but indicate 3-in-1 in the comments field. 

Rain Gear is provided to employees who are required to perform duties outdoors. Check with your supervisor prior to ordering rain gear to determine if the department has rain gear on hand.

Sizing:
Most uniform items are kept in stock at Administration. If you need to swap items out for a different size, please reach out to HR or Marketing to make arrangements.
