Nametag Order Form

Complete the form below to order your Zoo Staff Nametag. Please allow 5-7 business days for production.

Nametags must be worn by Zoo employees whenever working in a space where they may be interacting with Zoo guests.

By default, all staff will receive the smaller nametag without job title or department included. If your supervisor determines a need for that information to be included, please indicate that in the comments field on the form.

Employees are not required to include a last name on their nametag. Make sure to type your name exactly as you wish it to appear in the nametag contents.

Personal Pronouns
SCZ supports the option for employees to add personal pronouns to applicable branded identification. If you wish to include your pronouns, select your pronouns below.

If you do not wish to include your pronouns, simply leave the question blank.
